RESUMEN
La presencia de cuatro conductos radiculares es la configuración anatómica más común del primer molar maxilar permanente, pero otrasmorfologías también han sido descritas. El objetivo de este artículo es mostrar dos casos clínicos de primeros molares maxilares con la presenciade canales mesiopalatino y distopalatino. El uso de microscopio operatorio dental fue de gran ayuda a la hora de abordar los citados casos. Labúsqueda de configuraciones radiculares imprevistas es obligatoria.

ABSTRACT
The presence of four root canals is the most common anatomic configuration of the permanent maxillary first molar, but other morphologies have also been reported.The purpose of this report is to show a series of clinical cases of maxillary first molars with both mesiopalatal and distopalatal canals. The use of the dental operatingmicroscope was an important auxiliary when approaching the reported cases. A search for unexpected root canal configurations is mandatory. Awareness ofthese uncommon anatomic configurations should lead to better clinical outcomes.
